Medical Division

The Medical Division is responsible for determining the medical fitness for mariners to serve in the capacities specified by the domestic and international endorsements for which they apply.

To qualify for a medical certificate, a mariner must provide evidence of meeting the medical and physical standards on a CG-719-K or CG-719-K/E, as appropriate..

Medical Certificate

The medical certificate is a document that serves as proof that a mariner meets the required medical and physical standards. In order to receive a Merchant Mariner Credential or Medical Certificate you will need to complete the correct applications.

ATTENTION  :


  • MMC and Medical Certificates are processed separately.
  • E-mail Medical Certificate applications (CG 719K or K/E) to MEDAIP@uscg.mil.
  • Put LAST NAME_FIRST NAME_MARINER REFERENCE NUMBER in the subject line of the email to MEDAIP@uscg.mil.
    • NOTE: Drug test results are part of the MMC application and should NOT be sent with your Medical 719K application.
  • If necessary, E-mail medical related documentation (Additional Information) (not including applications for Medical Certificates) to NMCMedicalClerks@uscg.mil
  • Medical applications (719K) should be submitted as ONE PDF.  JPEG and other formats are not accepted.  Label your PDF as follows:  LAST NAME_FIRST NAME_REFERENCE NUMBER
  • Only COMPLETE medical applications are accepted for processing.
  • Please ensure all boxes on 719k are checked and that health care provider and mariner sign the document!
