The medical certificate is a document that serves as proof that a mariner meets the required medical and physical standards. In order to receive a Merchant Mariner Credential or Medical Certificate you will need to complete the correct applications.

ATTENTION :

NOTE: If you do not have a mariner reference number, label your PDF and subject line with: LAST NAME_FIRST NAME_ORIGINAL
MMC and Medical Certificates are processed separately.
E-mail Medical Certificate applications (CG 719K or K/E) to MEDAIP@uscg.mil.
Put LAST NAME_FIRST NAME_MARINER REFERENCE NUMBER in the subject line of the email to MEDAIP@uscg.mil.
- NOTE: Drug test results are part of the MMC application and should NOT be sent with your Medical 719K application.
If necessary, E-mail medical related documentation (Additional Information) (not including applications for Medical Certificates) to NMCMedicalClerks@uscg.mil
Medical applications (719K) should be submitted as ONE PDF. JPEG and other formats are not accepted. Label your PDF as follows: LAST NAME_FIRST NAME_REFERENCE NUMBER.
Only COMPLETE medical applications are accepted for processing.
Please ensure all boxes on 719k are checked and that health care provider and mariner sign the document!

CG-719K

Step 2 - Complete the Application for Merchant Mariner Medical Certificate (CG-719K).
Step 3 - Submit your complete form and supporting documentation in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available), including the subject line of the e-mail to: MEDAIP@uscg.mil . Make sure ALL boxes on the 719K are checked and that both applicant and the healthcare provider have signed the document. Only COMPLETE medical applications will be accepted.

CG-719K/E “Entry Level Only”

Step 1 - Review the Medical Certificate FAQ
Step 2 - Complete the Application for Merchant Mariner Medical Certificate for Entry Level Ratings (CG-719K/E “Entry Level Only” ).
Step 3 - Submit your complete form and supporting documentation in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available), including the subject line of the e-mail to: MEDAIP@uscg.mil. Make sure ALL boxes on the 719K are checked and that both applicant and the healthcare provider have signed the document. Only COMPLETE medical applications will be accepted.

Duplicate

Step 1 - Complete the Affidavit.
Step 2 - Duplicate, as follows: Submit the complete affidavit in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available), along with the word “DUPLICATE” and include in the subject line of the e-mail to: MEDAIP@uscg.mil.
Step 3 - (OPTIONAL) If you wish to receive your Medical Certificate via a method that can be tracked and will reach you in a few days, consider using an expedited mailing option expedited mailing option.

Additional Information (Existing Medical Certificate Application)

Step 1 - Subject line of your e-mail should be: Last Name_First Name_Mariner Reference Number (if available). For example: Smith, John, 1234567
Step 2 - Attach all documentation in PDF format. Other formats will not be processed.
Step 3 - Submit all documentation in PDF format in response to the letter to: NMCMedicalClerks@uscg.mil.
