APPLICATION FOR SIMPLIFIED MEASUREMENT
The Application for Simplified Measurement (CG-5397) is an Adobe Acrobat Form (.pdf) used to calculate tonnages under the Simplified Regulatory Measurement System. It can be used to apply to the National Vessel Documentation Center (NVDC) for tonnage assignment or as evidence of tonnage measurement for vessels ineligible for documentation.
For vessels intended to be documented, you may complete the form electronically, and then print it for signature, or you may print the blank form and complete it entirely by hand. Once completed, send the signed form to the NVDC by mail, fax, or as a “.pdf” attachment to an email. Forms intended only as evidence of tonnage measurement must be completed electronically and printed.

For example, some browsers display the form, but do not allow vessel data entry, or may allow data entry, but do not display calculated tonnages. If you encounter these issues, please download the form by saving the “.pdf” file to your computer, and open that file using your Adobe Acrobat software application.
Instructions
The calculated tonnages will be displayed in the upper left hand corner of the form, as indicated on the sample form on the right, when the necessary information required to perform the calculation has been entered. If the tonnages don't display, make sure you've hit the Enter key after typing the last entry. Note: It is recommended that the “Tab” button be used to progress through each of the form’s fields.
