• Step 1. Apply for your TWIC and complete the "application forms 719B, 719C (if applicable), CG-719S Sea Service (if applicable), and 719P” " using NMC’s Checklist Page to select the type of credential you want.
• Step 2. Obtain other documented Sea Service and any relevant training certificates and/or assessments, if applicable.
• Step 3. Pay your fees and submit all your forms, photocopies, and supporting documentation in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available), including the subject line of the e-mail to: MMCApplications@uscg.mil.MMC applications for Local Limited, Restricted, and First-Class Pilot MMCs, send to a: Regional Exam Centers.

Raise of Grade

Duplicate

Duplicate MMC definition:  A replacement credential issued containing the same authority, wording , and expiration date as a the lost, stolen, or destroyed credential.

Steps to Apply for a Duplicate MMC

• Step 1. Complete the 719B and affidavit.
• Step 2. Pay your fees.
• Step 3. Submit all your forms including affidavit in PDF format labeled with your : Last Name_First Name_Mariner Reference Number (if available)_DUPLICATE , including the subject line of the e-mail, and expedited mailing method (OPTIONAL) to MMCApplications@uscg.mil.

Replacement MMC

Replacement MMC definition: A replacement credential issued as the result of a request to change the name, address, or physical attributes on the credential.

Steps to Apply for a Replacement MMC

• Step 1. Complete the 719B.
• Step 2. Pay the modification fees.
• Step 3. Submit all your forms including supporting documentation and pay.gov receipt in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available) including the subject line of the e-mail and expedited mailing option to MMCApplications@uscg.mil.

Document of Continuity

• Step 1. Complete the CG-719B.
• Step 2. Submit all your forms, photocopies, supporting documentation in PDF format labeled with your Last Name_First Name_Mariner Reference Number (if available), including the subject line of the e-mail, and expedited mailing method (OPTIONAL) to MMCApplications@uscg.mil.

Additional Information (Existing MMC Application)

If you have received a letter requesting additional information for your MMC application:

• Step 1. Subject line of your e-mail should be: Mariner Last Name_First Name_Mariner Reference Number (if available). For example: Smith, John, 1234567
• Step 2. Attach all documentation in PDF format. Other formats will not be processed.
• Step 3. Submit all documentation in PDF format in response to the letter to: MMC-Awaiting-Info@uscg.mil.
