Office of Investigations & Casualty Analysis (CG-INV)

 

Organization:

 

The Office of Investigations & Casualty Analysis (CG-INV) reports to the Director of Inspections & Compliance (CG-5PC) and is located at the U.S. Coast Guard Headquarters building in Washington D.C. The office is comprised of three divisions:

  • Investigations Division (CG-INV-1)
  • Compliance Analysis Division (CG-INV-2)
  • Data Administration and FOIA Division (CG-INV-3)
In addition, the following Coast Guard Headquarters Units report to the Office:
  • Investigations National Center of Expertise, New Orleans LA
  • Suspension & Revocation National Center of Expertise, Martinsburg WV
  • Marine Safety Lab, New London CT

Mission:

 

We lead Coast Guard's investigation program to promote safety, protect the environment, and to prevent future accidents.

 

Captain Evelynn Samms serves as the Chief of the Office of Investigations and Casualty Analysis with Mr. Jason Neubauer serving as the Deputy Chief.  Commander Kelli Dougherty serves as the Chief of the Investigations Division (CG-INV-1). Commander Randy Waddington serves as the Chief of the Analysis Division (CG-INV-2).  Ms. Tolulope Olabode serves as the Chief of the Data Administration and FOIA Division (CG-INV-3).

 

 

 

Day-to-Day Activities and Key Processes:

 
  • Manage the marine casualty and accident investigation program.
  • Manage the civil penalty Notice of Violation (NOV) program.
  • Manage violation investigations; adjudicated by Hearing Officer.
  • Manage personnel action investigations program; prosecuted before Administrative Law Judge.
  • Conduct administrative clemency boards.
  • Manage the Drug and Alcohol Testing program.
  • Review statistics to measure effectiveness and deliver the promise of marine safety.
  • Support field in all of the above policy issues.
